REFERRALS & INSURANCE
We participate with most major insurance plans, including Medicare. However, it is your responsibility to understand any restrictions your policy may have and to obtain an insurance referral if necessary. Failure to do so may result payment for your claim to be denied leaving you responsible.
We do NOT accept Medicaid, WellCare, or new VA patients at this time.
We strive to provide the best estimated cost of all procedures with the information we are given. However, please note that these costs are only an estimate and your insurance determines the final amount once your claim has been processed.
All payments are due at the time services are rendered. By Federal Law and Managed Care Contract Law, this office is required to collect co-payments, co-insurance, and deductibles for each encounter. Any additional fees associated with the procedure/treatments will be collected following the appointment.
We accept cash, MasterCard, Visa, Discover, and CareCredit. We do NOT accept checks or American Express.
Working in close partnership with our referring doctors is an important aspect of the care we provide at Morrell Dermatology. We value each and every referral and take great pride in providing care to your patients.
You may refer patients to our office by downloading our referral form below. After you have printed and completed the form, please make sure to fax it along with any related documentation to our secure fax # 409-898-3901. If the patient is being referred for a skin cancer, please fax the pathology result and any other documentation such as pictures to prevent any delay in the patients care.
*Please note, if the referred patient has an HMO insurance we must have a copy of the insurance referral attached in order for the patient to be scheduled.
